I do a lot of fillable forms for my company as well and if you want to make them noticeable as fillable fields to anyone that opens the pdf i'd suggest creating boxes using text field properties, appearance, border color.there you can set your box color and thickness of line that will. I'm having the same issue.it's just stopped printing from the adobe viewer. It appears you already have fillable fields created from your screen shot. This is also where i got this template from.
